2025-2026 Student Health and School Forms 

 

Please note: All fillable forms must have an original parent/guardian signature; an electronic signature will NOT be accepted. By signing these documents, the parent/guardian certifies that they are authorized to complete the documents and that the information provided is true and accurate. 

 

 

The booklet version with these forms will be sent home after the start of the new school year.

Please, only submit each form once, per individual student.

 

Return all forms to students homeroom teachers.

• Family Income Information Form *
• School Messaging Consent Form (robo calls)
• Media Consent Form and Release
• Directory and Recruiter Opt-Out Form
• Student Medical Information Form
• Healthcare Provider Statement for Food Substitution
• State of Illinois Eye Examination Report Form (only needed upon enrollment)
• Proof of Dental Examination (Kindergarten, 2nd grade & 6th grade only)
• Vision Exam Consent Form (optional - only if services are needed) *
• Dental Examination Consent Form (optional - only  if services are needed) *
