New Student Reg. Forms

Please note: All forms completed online must be downloaded, printed and have an original parent/guardian signature; an electronic signature will not be accepted. By signing these documents, the parent/guardian certifies that they are authorized to complete the documents and that the information provided is true and accurate.

++++ Forms must be printed and have a physical signature before they are delivered to the office ++++

 

#### The Home Language Survey is a doubled sided form and both sides must be completed in full in order to be considered valid. Otherwise, it will not be accepted and we will require it to be resubmitted. ####

 

 When you return your completed forms to the office, please remember to bring:

 

· Student’s Original Birth Certificate

· 3 Forms of Proof of Residency which may include:

· current billing statements - utility, medical, cellular, KidCare statement

· current bank or mortgage statement

· Current medical forms including an annual physical (with proof of immunizations), dental and vision (due no later than Oct. 1st).

 

**** Lease Agreements and Driver’s Licenses will not be accepted as Proof of Residency ****
